DANDA #021 — ScriptWise: AI Agents to End Prescription Confusion and Medication Errors

Every day, millions of Americans pick up prescriptions—yet 1 in 5 patients receives conflicting medication instructions, duplicates, or dosing errors. Despite digitization, prescription confusion drives preventable ER visits, hospital stays, and even deaths. It's not just a paperwork problem: medication mistakes cost the US $528 billion every year and are responsible for more than 4.5 million annual hospitalizations. As drug regimens multiply and healthcare fragments, patients and providers are overwhelmed with disparate systems, missing information, and zero-check accountability.

The problem

Medication errors are one of the most expensive and deadly hidden problems in healthcare. In the US alone, there are 7 million preventable medication errors annually. 1.5 million Americans are harmed each year by drug mistakes, and 4.5 million are hospitalized because of adverse reactions. The financial toll is staggering—$528 billion every year, equal to 16% of total US healthcare spend. Most errors arise from prescription confusion: duplicate therapies, conflicting instructions, missed allergies, or failure to reconcile new medications.

The idea: ScriptWise

ScriptWise deploys AI agents directly in pharmacies, clinics, and patient apps to clarify prescriptions, detect conflicts, and reconcile medication lists in real time. Each agent automatically ingests new scripts, cross-checks against patient profiles, provider notes, and pharmacy records, and flags issues instantly—duplicate therapies, dosing confusion, interactions, allergy mismatches. For patients, ScriptWise delivers clear, personalized instructions and reminders. For providers and pharmacists, it gives actionable alerts and automated reconciliation workflows. The result: fewer errors, safer outcomes, and less wasted spend.

Architecture

Inputs Rx, EHR, Pharmacy Ingestion Parsing, Normalization Memory / Graph Patient, Drug, Allergy Agent Orchestrator Logic, Reconciliation Human Gate Provider, Pharmacist Action Layer Alert, Instructions

ScriptWise starts by ingesting prescription and patient data from EHRs, pharmacy systems, and direct uploads. The ingestion layer normalizes formats and parses key drug, dose, and allergy info. A memory graph organizes patient-specific medication history and known risks. The agent orchestrator applies rule-based and model-driven reconciliation, flagging conflicts or errors. Human gate ensures oversight: providers and pharmacists review AI-generated alerts. Action layer delivers targeted instructions and notifications—patients get clarified dosing, providers get actionable alerts, and pharmacies get reconciliation reports.

Build plan (90 days)

Wedge: Start with independent pharmacies (15k in US) where medication errors and reconciliation are most acute. Stack: Integrate EHR and pharmacy data ingestion, build patient memory graph, deploy agent orchestrator, and roll out provider/pharmacist review flows. Pricing: $99/month/location SaaS for pharmacies; $2/month/patient via clinics; free tier with in-app purchase for patients.

Why now

Medication complexity is exploding—average patients now take 5+ drugs, and digital prescription volume doubled since 2020. Yet fragmentation and staffing shortages make reconciliation impossible by hand. AI can ingest, cross-check, and clarify at scale, slashing preventable errors and unlocking billions in healthcare savings. ScriptWise's timing is perfect: EHRs can now connect, regulations demand reconciliation, and everyone—from patients to providers—is demanding safer clarity.
