DANDA #082 — ClaimCare AI: End Healthcare Prior Authorization Hell for Clinics & Patients

Every day, doctors and clinics across the U.S. waste precious hours on 'prior authorization'—the Byzantine process where insurers force providers to submit mountains of paperwork to approve essential tests, medications, or surgeries. Patients suffer as care is delayed, sometimes for weeks, while clinics burn staff time on mindless forms and endless phone trees. The result: unnecessary illness, wasted money, and a system that puts bureaucracy before health.

The problem

Prior authorization is the #1 cause of admin burnout in healthcare. Clinics spend an average of 13 hours per physician per week—over 1.1 million hours daily nationwide—on prior auth admin. The AMA found that 94% of doctors report treatment delays due to prior auth, and 33% say patients frequently abandon care because of it. For patients, a single request can mean up to 3 weeks of waiting. The cost: an estimated $35 billion/year in wasted labor and an incalculable human toll as patients delay or forgo vital treatment.

The idea: ClaimCare AI

ClaimCare AI is an autonomous agent that takes the entire prior authorization burden off clinics and patients. It ingests EMR data, reads and structures order details, pulls the right payer forms, auto-populates every field, tracks status, and escalates denials. With real-time API and RPA integrations to insurer portals, it relentlessly pursues approvals—flagging only edge cases for human review. Clinics go from hours of paperwork per order to minutes, patients get care days or weeks faster, and insurers see fewer errors and appeals.

Architecture

Clinic EMR Insurer Portal Ingestion API & OCR Patient Case Graph Memory Agent Orchestrator PA Engine Human Gate Review / Escalate Action Layer Submit & Track Inputs Ingestion Memory/Graph Agent Orchestrator Human Gate Action Layer

The flow: ClaimCare AI ingests clinical orders straight from the clinic’s EMR and the insurer’s portal. The ingestion layer parses structured and unstructured data using APIs and OCR. The patient’s case—including diagnoses, past auths, and payer rules—is dynamically modeled in graph memory. The Agent Orchestrator applies payer-specific logic to fill, submit, and track prior auths, sending complex or ambiguous cases to the Human Gate for review. The Action Layer actually files requests, monitors status, escalates denials, and syncs outcomes back to the EMR, closing the loop for the clinic and patient.

Build plan (90 days)

Wedge: Target large specialty clinics (cardiology, oncology) seeing >50 prior auths/week. Integrate with Epic and Athenahealth EMRs; launch with 3 top payers covering 60%+ of insured patients. Stack: (1) API+RPA ingestion for EMRs/portals, (2) payer logic engine, (3) self-improving denial/escalation agent, (4) user dashboard with audit trail. Pricing: SaaS, $499/mo per provider seat, plus $2 per approved request—clinics save 10x on admin labor alone.

Why now

Payer automation APIs are finally rolling out. GenAI can parse clinical doc chaos and output perfect forms. Clinics are desperate: admin costs outpace physician pay, and prior auths are rising 15% per year. Patients, providers, and regulators all want this nightmare gone. ClaimCare AI can end one of care’s worst bottlenecks—now, not next decade.
